Social Prescribing

The Pier Health Social prescribing team began supporting clients in 2020. They are a partnership between Citizens Advice North Somerset, Alliance Homes and The Pier Health partnership. 

What is social prescribing?

Social Prescribing connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing. They work with adults 18+.  

In the Pier Health Social prescribing team, they follow a what matters to me approach and the 5 ways to wellbeing. 

Your link worker will work with you to prioritise what matters to you and changes you want to make in your life. They can help you set goals and formulate a plan to taking small steps to achieving them.

Five Ways To Wellbeing

The Five Ways to Wellbeing are a set of evidence-based actions that promote mental and emotional wellness: 

  • Connect: Foster meaningful relationships. Social interactions with friends, family and community build and support networks, reducing feeling of isolation.
  • Be Active: Engage in regular physical activity. Exercise releases endorphins, improving mood and overall health, while boosting self-esteem.
  • Take Notice: Practice mindfulness. Pay attention to the present moment, enhancing awareness, reducing stress and encouraging a deeper appreciation of surroundings.
  • Keep Learning: Continuously engage in new learning experiences. Acquiring new skills and knowledge boosts self-confidence, stimulates the brain and promotes personal growth.
  • Give: Contribute to others. Acts of kindness and volunteering create a sense of purpose, increase self-worth, and enhance social connections, thereby improving mental wellbeing.

Who do social prescibing support?

  • People over 18 years old   
  • People that have one or more long-term conditions     
  • People that need ‘low-level’ support with their mental health     
  • People that are lonely or isolated    
  • People that have complex social needs which affect their wellbeing.

How do you get a referral?

Referral is through your Pier Health GP Practice.

Your GP refer you but you are more than welcome to self-refer, please speak to a member of our reception team to self-refer into social prescribing.
